I've had shoulder problems for years and could voluntarily subluxate my right shoulder and wing my scapula. Several physios could do nothing. One orthopedic surgeon wanted to surgically shorten the ligaments, an idea I was not too keen on. Eventually a friend with shoulders worse than mine recommended a physio who specialised in shoulders and I finally received the proper rehab. So the trick is finding a physiotherapist who specialises in shoulders. Maybe try contacting some of the local professional sporting clubs and see where they send their injured athletes.

Check out Eric Cressey's articles, here's one, for example. He works a lot with Mike Robertson, one of the co-authors of that TNation article above. Apparently he is THE shoulder guy among the new wave of strength coaches. Not only is he very extensively published (in legitimate, peer-reviewed journals) but he also has a 295 deadlift at 82 kg bodyweight...

He does "e-coaching" but it's pretty expensive. If you have insurance I'd go with a competent physio shoulder specialist, as others have mentioned.

The press-up variation mentioned by thestoatyone is a good one. If you google for "pushup plus", you should find some demos. Actually, you can leave out the pushup and do the forward shrug on its own. Focus on keeping your elbows fixed in one position throughout the whole movement, though not necessarily locked out completely.

If you want to make it harder, there are several possibilities:
1) Use antagonist contraction - I think we have talked about this before.
2) Put some weight plates on your back.
3) Take a rubber tube in both hands and wrap it around your upper back. If you do the exercise in a reha setting, these thingies should be available.

And finally, if your concern is not just functional but also aesthetic - posture and low BF are more important than isolated training for making these muscles visible!

Pushups. Pushups improve that muscle, try doing different ones--military, wide, single leg, plyo...go at it!
